How to Check Your Insurance Benefits

Contact your insurance company by calling the number on the back of the card and ask:

• Do I have a co-pay?

• Is prior authorization required? 

• How many sessions are allowed per year?

• Do I have a deductible to meet before the insurance will pay for services?

Out-of-Network Benefits

You can call your insurance company to ask if you have out-of-network benefits. I can provide a Superbill which you can submit to your insurance company for partial reimbursement. I cannot guarantee reimbursement from your insurance company.

Questions you can ask about your out-of-network benefits:

• Do I have an out-of-network deductible?

• How much of my deductible has been met this year? 

• What is my out-of-network deductible for outpatient mental health?

• Do I have co-insurance? What is my out-of-network deductible for mental health?

• Do I need a referral from an in-network provider to see someone out-of-network?

• How do I submit claims for reimbursement?

What is a Good Faith Estimate?

You have a right to receive a Good Faith Estimate of expected charges Under Section 2799B-6 of the Public Service Act. A "Good Faith Estimate" is an estimated bill for services so you understand how much your mental health care will cost. This applies to any individuals who are not enrolled in insurance or who are not going to file a claim with their plan for coverage or reimbursement.

Financial Practices

Co-Payments and Payments are due at the time of service.
Full financial practices and policies will be provided to you for acknowledgment and signature at intake.
Cash, check, and all major credit cards are accepted, including HSA/FSA cards.
